This is a point to point, easy to use region-wide teleporter.
Rez 2 pads where you like on the same sim (even high on the sky), and it's done!
No notecard required.
If you want to move the pads to a different position, just do that: the 2 pads communicate their respective positions to each other and update their teleport coordinates automatically,
The pads are 1 prim each and are copy+mod (the script is copy only)
Of course you must have the permission to run scripts where you rez the pads.

Final Conclusion
We used it a few weeks just to get to the roof of our condo-tower. Put them down and mostly done. If you have to get somehow in your skybox and you just have a small parcel, consider this teleporter.

Brilliantly easy
After years of fiddling around with notecards, it was great to come across this handy teleporter - brilliant for single point-to-point teleporting.
