If you right click an alpha layer from inventory and choose "add" from the drop down menu it will combine it with any alpha layers you are currently wearing,such as clothes & shoes for example.
How do I wear the mesh item?

It's really simple! you can double click the item from your inventory.
Additionally, you can right click the item in inventory and choose 'wear' or 'add.'
Wear will replace any other object/mesh attached to the same point on your avatar.
