NOTE: Sometimes when its worn, it will look fragmented to you or others. (I've seen it a few times and so have others. Its a SL glitch. ) This isn't the mesh, its a viewer issue, just detach and re-attach the mesh usually fixes it.
Also you can go into debug settings. (ctrl+alt+shift+s) and type in MeshMaxConcurrentRequests. Set that value to something higher. I have mine at 256.
