Your Visitors can leave you feedback, apply for a job, or take a survey and your staff members can access the Notecards dropped in the box from Touch Menu!
Visitors Touch the box to get your Blank Form that you setup with your questions and information. After they fill out their information they drop this Notecard back into the object.
It can be set to notify the object owner and up to 3 other avatars by IM when a new note is dropped in.
You set up an Access List with the names of your staff members you wish to grant access to the Notecards received. These "Managers" can Touch the object for a special Dialog Menu with options for "Get all Notes," "Delete all Notes," and more.
Visitors will only be able to add Notecards. All other inventory types will automatically be deleted. (Items added by object owner are not deleted.)

it works
but very poorly, like for instance i was testing it out with my alt and it said i had -1 notecards so i had to drop it in twice, another time i did it it said "please use only notecards" which i was doing, and finally when you go to retrieve the notecards you get a "new notecard 1" i dunno why but its far from flawless
