Bought twice - Have not been able to rez it

This seems to be a one-time use object. When I try to rez it, I only get options to add it to my outfit. And since I blew that, its gone from my folder.

Bought it again, tried again. No joy.

I am new, so I may be doing it wrong, but still - instructions unclear.
