Add it on the avatar. By default it attaches to the mouth. Touch the avatar's head to see the menu and select an emoticon.
You can additionally use keywords instead of the menu to activate an emoticon by using the channel /1 (example: /1 :D, /1 :(, etc.)

This is an awesome item.
I found that when trying to concentrate with your avatar dancing at a club; it keeps disappearing, and the only way it can be made to re-appear is to click the hud again. This item needs the option to switch it on permanently. Maybe that can happen in an update.
