You may need to edit your avatar's hover height after attachingas this is meant to put you at a more average height with other avatars, 56 worked for me so it's not much of a change. Default attachment point is the Pelvis. You will also need the AO Orthopedics Wheelchair Dance AO to make the movement and dance effects look right, it's free and can be found here http://maps.secondlife.com/secondlife/Tipsico/116/23/159
You will also need to hide your avater below the waist with either an alpha layer (included) or hud if you have a mesh avatar
