OFEQ Vortex Teleporter - Capable of teleporting to altitudes up to 4000 meters in the sky! (See notes below)

Go to the locations where you would like to teleport, and rez a teleporter. Give the teleporters a few seconds to detect each other, and you're done. Touch a teleporter to bring up a list of destinations. After you select your desired destination, a teleport beam will appear. Sit on the beam and teleport to the destination. If you would like, you can change the name of each teleporter to reflect the destination. If you do not name them yourself, they automatically name themselves based on the parcel name and their region coordinates.

Note: At the time of this writing, some SecondLife clients do not support editing/moving objects over 768 meters in the sky. Although the teleporter can take you to and from these altitudes, if you try to move your teleporter at an altitude above 768 meters, it may immediately snap down to 768 meters. If you've tried to do this and your teleporter has disappeared, try flying down to 768m to find it. Remember, if you want a teleporter higher than 768m, rez it precisely where you want it. You may not be able to move it using older SecondLife clients at these altitudes.

If you need a teleporter with less prims, try the OFEQ Teleporter (LE).
