what you get:

system eye (copy only)
This is the normal avatar eye file and can be worn by itself.

mesh eyes (copy/modify)
Wear these for higher definition. They are modifiable, so you can turn on fullbright or glow or tint them or add particles or whatever. For best results, wear the matching system eye at the same time.

eye alpha layer
You shouldn't really need this, but it's a cool thing that makes the normal avatar eyes disappear. If your LOD settings are low, the mesh eye might appear to flicker. Wearing the system eyes along with the mesh minimizes this, but if it's really bad, apply the alpha (just ADD like any other alpha layer).
