A clockwork heart, cracked and rusted from years of abuse, disuse, or perhaps overwinding. Attaches to the chest.
The scripted version plays a Tick-Tock when you walk, as well as when touched. Also types in local chat:

"(person touching) tinkers with (your name)'s gears." or "(person touching) winds the spring on (your name)'s clockwork heart." when touched by someone else
