HOW IT WORKS:
Avatar clicks the object, gets a menu with your question, answers by pressing one of the buttons (MAXIMUM 3 BUTTONS), and receives "thank you" in local chat! What script actually does are 3 things:
A) in simplest possible way, enables you to format the dialog menu message to look as you desire;
B) records clicks on the buttons;
C) at any time gives a clear report on touch in local chat, only to you (owner), on how many times each button was pressed!
ADVANTAGES:
• you can start collecting useful statistics within minutes!
• question/message/request/suggestions can be formated anyway you like: with or without numbers, line brakes or empty lines, you can make it look professional, or funny!
• no notecard or script configuration required (edit object's name and description for dialog message)!

• user-friendly for both you and residents: only touch activated (no laggy sensors), anonymous;
• secure: one avi can click only one button only once and is never presented with the menu again; reports are saved even if you take the object to your inventory and rez it again, only manual script reset will delete previous results!

No Notecard to rename buttons
Instructions state the following to name/add buttons for responses...
3. !!! Open the NOTECARD in the contents of the Surveyor, and name the buttons (button labels) by putting a list of words separated by comma. EXAMPLE:Google,Microsoft,Apple,Youtube,Flickr,Pinterest,Instagram,Tweeter,Facebook,SecondLife,MARKETPLACE,taraFAB :) , and save it. You don't have to use letters, you can put some ASCII symbols, but they take more space, and some probably wont' work. Maximum number of buttons is 12 (minimum is 1, but if you want, for example 5 buttons, only 5 buttons will be on the menu), and maximum number of characters for each button is 11 (if you get an error, make the label shorter), including spaces. You can rename the notecard however you want. That's it, you're good to go!
Problem is .. no notecared is included to OPEN??? And when you create your own and name it whatever you want as the instructions clearly say?? Nothing happens and it doesn't work.
