Edit Suggestions Box, Select Content Tab, Edit the notecard to mention your name, store name, and list any information you want people to include, then save your note card.

Or create a new note card so you are the creator. then rename the note card to "Suggestion Card"
Delete the old Suggestion Card from suggestion box, then add yours. test it by clicking non suggestion box. if you get the card you did everything right. if you get a script error you may have named your note card incorrectly.

What people drop a notecard into the box it will play a cute sound and generate some confetti for 2 seconds. Then it will contact you if you are online. You may also want to check the box when you log in.
