Simple script for Sound on collision + Animation option

It took me forever to figure out how to get the sound to work, and when I enter the animation name, it does not trigger the animation. I am not a scripter, which is why I opted to buy a script... but I am very disappointed, that is all.

I'm sure it works.. but no instructions
Please give me instructions so I can change this vote.. for those that don't know about bumper scripts, we buy it because we can't write them.
