Forms & Surveys Agent (FSA) posts to a user a series of questions from a preconfigured questionnaire, processes answers and emails completed entries to specified addresses.
A user may be paid for completion or rewarded by getting items from the FSA content. The FSA keeps a log of the most recent 500 usernames and may be configured to either allow or prohibit multiple entries by the same user.
A questionnaire may be anything: an application, a form, a survey, a test, etc. Switching questionnaires is as simple as replacing a notecard in the FSA content. In the questionnaire notecard you enter your questions in plain text and their answer formats in a simple yet powerful mini-language. There may be multiple-choice answers, text answers, numeric answers, you may even alter the questions flow depending upon answers to previous questions.
This unique flexibility combined with a rich set of features makes the FSA an invaluable tool for taking applications, conducting surveys or testing students.
