Forms & Surveys Agent (FSA) posts to a user a series of questions from a preconfigured questionnaire, processes answers and emails completed entries to specified addresses.
A user may be paid for completion or rewarded by getting items from the FSA content. The FSA keeps a log of the most recent 500 usernames and may be configured to either allow or prohibit multiple entries by the same user.
A questionnaire may be anything: an application, a form, a survey, a test, etc. Switching questionnaires is as simple as replacing a notecard in the FSA content. In the questionnaire notecard you enter your questions in plain text and their answer formats in a simple yet powerful mini-language. There may be multiple-choice answers, text answers, numeric answers, you may even alter the questions flow depending upon answers to previous questions.
This unique flexibility combined with a rich set of features makes the FSA an invaluable tool for taking applications, conducting surveys or testing students.

The creator really understands how SL works and makes it easy for people to interact. People generally don't like to type or write much (and they're also not into reading notecards, lol). This board comes with simple steps, allowing people to just click to answer.
I use it for questionnaire and member application. We can see that it's more active activity than just asking people to write notecard and drop

Most useful item I've bought in SL, and most versatile!
Very well written and bug free. Because this item comes with a meta language capability, you can write your own apps to do a wide variety of things other than just surveys. In effect the simple language does conditional branching, allows several question and answer formats, is quick and easy to learn and code.
It has a huge number of options (sent results to email, don't let person take survey twice, time out if person quits in the middle, too many more to mention!). I bought this to do surveys but it does so much more. It completely replaces the single person simple mailbox I was using to get various feedback types from my 740 group members, is much more likely to be used than making them fill out a notecard, and best of all, it gets their name and the format RIGHT every time vs a scribbled nc method which was a time consuming disaster to fix each card.
Then it sends the results to one or more emails meaning you get the info automatically as well as your chosen group officers if you want that.
If THAT'S not enough, the images are copy and you can even put your own signature brand on the open for use and in use screens. Customer service is fantastic too! Would and DID buy again!

I looked at a lot of form scripts on sl before coming across this one .. Even though I already bought few other scripts , this was the best one ... customizable according to your needs ...set your own questions, time duration , email configuration ..It had everything I can ask for .
Really good product to conduct surveys , feedbacks , conduct quizzes, class tests or in different roleplays .. I have already forgotten google forms ..lol
Other than this , a really good thing about this product is amazing customer support . I asked for support from Ela and got a reply within no time .She helped me and been patient with me as I was really making silly mistakes and lastly configured it herself for me . Thanks Ela !
